Administrative Forms

In this section you can find the majority of forms that we use in the Boulder Creek Recreation District. We are gradually moving to using the Abode PDF format for all new and future forms and notices. You can obtain the free Adobe Reader software by clicking the graphic at the bottom of this page. Should you need the form in another format please send an email to the webmaster and I'll attempt to provide the form in your requested format.

Rental Agreement Form
	Rental agreement form for use of BCRPD facilities and parks. PDF (8KB) Anybody renting the use of BCRPD facilities and/or parks must return a dated and signed agreement form before use.
Program and Activity Forms
	Program registration agreement form with waiver and release. PDF (7KB) All program participates must return a dated and signed agreement form before attending any BCRPD programs.
Instructor and Program Leader Forms
	Proposed class form. PDF (6KB) Any instructor proposing a course or activities for the District must fill out and submit. Please call the Program Manager at (831) 338-4144 for more information.
Team Registration Forms
	Team registration agreement form with wavier and release. PDF (8KB) All team members and players must read and sign before commencing any team activities at any BCRPD facilities and/or parks.
